AOA,
My name is Khurram Shahzad I am working in a medical billing company in Islamabad. We deduct withholding tax on salaries of employees on monthly basis. I want to learn about depositing withholding tax i-e

1- Where to get the payment form ( I have searched few forms on FBR website but cannot decide which form is exactly required)
2- What is the rate of withholding tax deduction
3- I have heard that there are 4 copies of withholding tax deposit receipt( Original, duplicate, triplicate and Quadri..) which of these copies of receipt do I need to keep with myself and which one of these are required to be submitted into FBR?
4- If I need to submit one copy to FBR please guide me how to submit it?

Just tell me complete procedure, Thanks
